Return Materials Authorization Request Form

Please fill out this form to request a Return Materials Authorization (RMA).

Products to be returned for warranty coverage must be within the applicable warranty period and meet Amerex warranty requirements. Amerex will provide a return label for all returns that fall under the warranty policy. If the product is not covered by the Amerex warranty, 25% of the purchase price will be applied as a restocking charge and the customer will be responsible for return to the designated warehouse within 30 days.

If products arrive damaged, the customer must notify Amerex within 3 business days before returning it to ensure proper handling. The customer should keep the original packaging, including the shipping box. If items are missing from the customer’s shipment, the customer must notify Amerex within 5 business days.
